chore(wolf): update session tracking state

Browse source 
anatomy.md/cerebrum.md/memory.md/token-ledger.json/hooks state covering
the design-review fix pass (tlog activation, core-isolation audit,
schedAdmin admission gate, resource-value-verification doc/comment gaps)
and task 12.5's fresh completion verification.
