The buglog was one 27K-line JSON file whose fields covered only symptom,
root cause and fix, as free text: 2% of entries named the code version a
bug was found on, 12% the build profile, 36% a way to reproduce, and 58% of
the file was OpenWolf hook auto-detections ("Missing guard clause",
"Function not marked async" on .md files).
- .claude/skills/buglog/bug-record.md: the record standard. Eight
sections (Summary as the title, Description, Environment, Reproduction;
Root cause, Fix, Verification, References once fixed), each with the
content it must contain -- e.g. Environment needs the OS, the libc the
failing binary loads (from its ELF interpreter: glibc 2.42 in the nix
shell vs 2.40 on the host), toolchain, build profile and commit.
- .wolf/bugs/<id>.md: 500 records migrated from buglog.json; every field
verified to round-trip. They carry legacy: true and an explicit "not
recorded" where the old log had no such field; component derived from
file paths, severity from keywords (else unknown), fix_commits from
hashes in status_note/fix that git resolves (91 of 463 fixed).
- .wolf/buglog-archive.json: the 710 hook auto-detections, verbatim.
- .wolf/buglog.json is now an index generated from the records, keeping
the fields OpenWolf's hooks and `openwolf bug search` read.
- scripts/buglog.py rewritten: new (template + Environment collected from
the machine and --binary), check, status (refuses a status the record
doesn't support; fixed needs Root cause, Fix, Verification and
--commit hashes git resolves), note, recur, set, show, list, triage,
env, index. 29 unittest cases in scripts/tests/test_buglog.py.
- scripts/git-hooks/pre-commit runs `check --staged`: incomplete staged
records, a legacy record hand-changed to fixed without the fix-time
sections, a new record claiming legacy: true, or a stale index refuse
the commit.
- scripts/gitea-sync.py reads the records: issue body = the record's
sections plus linked fix commits (origin remote, GITEA_COMMIT_REPO to
override), and a body hash so an issue's text is refreshed when the
record changes.
- bug-1866 rewritten to the full standard as the worked example.

bug-849: After bug-848 fix, restart still loses the committed prefix: restarted ctrl-1 ended with WAL…
Description
After bug-848 fix, restart still loses the committed prefix: restarted ctrl-1 ended with WAL [15..16] only — missing [1..14] (waldump 'GAP: missing prefix LSN 1..14'). Payloads now full (bug-848 era), but the prefix is gone. Loss mode changed (empty-payload -> missing-prefix), loss remains.
Environment
Not recorded (migrated from buglog.json).
Reproduction
Not recorded (migrated from buglog.json).
Root cause
CONFIRMED from live logs (/tmp/xce-ha-deploy/logs/ctrl-1.log). persist_recover (and the bug-848 applied_lsn sync, and catch-up) run ONLY inside cluster_init_phase2, which is ELECTION-GATED — its real callers are all ha.c election/promotion paths (ha.c:1021,1166,2943,3022) + cluster_init. First boot ran it (ctrl-1 won an election; logged 'persist_recover done — durable_floor=0 current_lsn=1'). The 11:07:20 RESTART came up as a follower ('myapp_run: CTRL_STANDBY — waiting for primary to connect and push config') with NO election → cluster_init_phase2 NEVER ran → NO 'persist_recover done' line for pid 304897. So the rejoining follower never recovered its on-disk [1..7], operated with current_lsn=1/applied_lsn=0, then received LIVE replication of the new job [15..16] (repl_broadcast), O_TRUNC'd seg-001 and wrote [15..16] there. [1..7] never recovered + [1..14] never caught up = permanent prefix gap. Ordering corollary: live replication advances applied past the gap so the primary's catch-up never backfills it.
Fix
FIXED 2026-06-21 (design B — boot-path-universal recovery). Refined root cause: phase2 is invoked ONLY from the election-result path; a rejoining follower that SKIPS its election (goto skip_election in myapp_run because a primary already exists) never reaches it. Fix: in myapp.c the skip-election standby branch now calls cluster_init_phase2(&c) (build resources + persist_recover + bug-848 ha_sync_applied_lsn) when !ha.phase2_done, BEFORE cluster_run starts serving — so every boot path recovers its own WAL and advertises its real applied_lsn before accepting replication. Idempotent via phase2_done (same once-per-process contract as the election callers); recovery decoupled from the primary config push (a restart already loaded config from disk pre-election). Daemon builds; controller suite incl. test_persist_recovery/test_ha_startup/test_repl_catchup/test_ha_failover 8/8 green. LIVE VALIDATION PENDING: redeploy + kill/restart ctrl-1; expect a 'HA: persist_recover done — durable_floor=7 current_lsn=8' line for the restarted pid and ctrl-1 keeping [1..7] + catching up only the suffix.
