The buglog was one 27K-line JSON file whose fields covered only symptom,
root cause and fix, as free text: 2% of entries named the code version a
bug was found on, 12% the build profile, 36% a way to reproduce, and 58% of
the file was OpenWolf hook auto-detections ("Missing guard clause",
"Function not marked async" on .md files).
- .claude/skills/buglog/bug-record.md: the record standard. Eight
sections (Summary as the title, Description, Environment, Reproduction;
Root cause, Fix, Verification, References once fixed), each with the
content it must contain -- e.g. Environment needs the OS, the libc the
failing binary loads (from its ELF interpreter: glibc 2.42 in the nix
shell vs 2.40 on the host), toolchain, build profile and commit.
- .wolf/bugs/<id>.md: 500 records migrated from buglog.json; every field
verified to round-trip. They carry legacy: true and an explicit "not
recorded" where the old log had no such field; component derived from
file paths, severity from keywords (else unknown), fix_commits from
hashes in status_note/fix that git resolves (91 of 463 fixed).
- .wolf/buglog-archive.json: the 710 hook auto-detections, verbatim.
- .wolf/buglog.json is now an index generated from the records, keeping
the fields OpenWolf's hooks and `openwolf bug search` read.
- scripts/buglog.py rewritten: new (template + Environment collected from
the machine and --binary), check, status (refuses a status the record
doesn't support; fixed needs Root cause, Fix, Verification and
--commit hashes git resolves), note, recur, set, show, list, triage,
env, index. 29 unittest cases in scripts/tests/test_buglog.py.
- scripts/git-hooks/pre-commit runs `check --staged`: incomplete staged
records, a legacy record hand-changed to fixed without the fix-time
sections, a new record claiming legacy: true, or a stale index refuse
the commit.
- scripts/gitea-sync.py reads the records: issue body = the record's
sections plus linked fix commits (origin remote, GITEA_COMMIT_REPO to
override), and a body hash so an issue's text is refreshed when the
record changes.
- bug-1866 rewritten to the full standard as the worked example.

bug-605: 3-node fresh HA deploy never elects a primary: all nodes stay CTRL_STANDBY, config-holder (ctrl-1)…
Description
3-node fresh HA deploy never elects a primary: all nodes stay CTRL_STANDBY, config-holder (ctrl-1) loops PreVote LOST every ~11s forever; config-less peers idle as voter-only. REPRODUCED via /xce-deploy-ha + clean restart 2026-06-16.
Environment
Not recorded (migrated from buglog.json).
Reproduction
Not recorded (migrated from buglog.json).
Root cause
ha_dispatch_gate() admitted OPCode_requestVote in CTRL_INIT/CTRL_ELECTING/CTRL_STANDBY but NEVER OPCode_preVoteRequest (opcode 38). ha-prevote added the new opcode + handler (cluster.c:440) but missed wiring it into the role-admission gate. So a STANDBY voter rejected an incoming PreVote with -XCE_ERR_STANDBY (-10014) BEFORE the handler ran -> no preVoteReply sent -> candidate's failed/empty exchange tallied as a denial (pre_vote_reply_decode_and_dispatch counts ok=0 as non-grant; vote_ep_err_handler synthesizes a denied vote). In a cold cluster (config-holder + config-less voter-only peers) every PreVote 'lost' with NO voter ever making a liveness decision (zero 'PreVote DENIED' logs), so no primary was ever elected and the config-holder looped PreVote forever. Confirmed via gdb: myreq_handler ha_dispatch_gate rc=-10014 on opcode 38. Design invariant: a PreVote (strictly non-mutating) must be at least as admissible as a real requestVote in every role.
Fix
Added OPCode_preVoteRequest alongside OPCode_requestVote in ha_dispatch_gate() for CTRL_INIT, CTRL_ELECTING, and the CTRL_STANDBY switch (src/controller/ha.c). PreVote now reaches the cluster.c handler and a proper preVoteReply is sent. Added TDD test test_dispatch_gate_allows_prevote_in_voter_roles (test_ha_role.c) asserting gate(preVoteRequest)==gate(requestVote)==0 across voter roles. VERIFIED: controller unit suite OK (new test PASS); fresh /xce-deploy-ha 3-node cold start now elects ctrl-1 PRIMARY (term=2), standbys establish primary channel + config committed, prevote-LOST=0, connections 96->8.
